Adults and wellbeing

The adults and wellbeing directorate has brought together adult social care, strategic housing and public health into a single directorate. This approach is in line with our customers' experiences and makes our commissioning and service procurement more effective. Our approach is based on personalisation in order to deliver better outcomes for people that help build resilience and give people greater control over their lives.

Our teams and the areas they cover are:

Operations and support

  • Advice and referral team (ART)
    The advice and referral team is our front door to adult social care. It is the gateway to services provided by ourselves and our partners.
  • City, East and West locality social care teams
    These teams support vulnerable adults, helping them to live safely, independently and within their own communities. The services also include prevention of admission and support with discharge from hospital.
  • North and South social teams (complex needs and Deprivation of Liberty Safeguarding (DoLS))
    The North and South teams work with adults who need longer engagement and more specialist services.
  • Prevention and support
    The teams involved in prevention and support include telecare, home improvement agency, welfare, assessment, finance and advice team, homelessness and direct payments.

  • Contracts, quality and review
    Our directorate has a new Quality Assurance Framework which complements our contract monitoring and has boosted relationships with providers. 
  • Research and intelligence
    The research and intelligence team helps to make sure we have evidenced-based practice and that decisions taken are based upon robust data.
  • Commissioners
    We have created flexible structures in commissioning to help us embrace developments allied to the Better Care Fund and to make the best use of available resources. 
  • Brokerage
    Including brokerage in commissioning helps us ensure we have a whole system and process in placed that is based upon the commissioning cycle.

    ASCOF returns and continued performance monitoring to check progress against targets.
  • Prevention and demand management
  • Information and guidance
    Supporting the prevention agenda by using innovative design to change cultures and create community connectivity. We also embrace the use of WISH, Herefordshire's new information, wellbeing and signposting service, as a key thread in all the transformation work we do.
  • Project management (transformation and engagement)
    Supporting commissioners and operations teams by project managing specific pieces of work. This typically includes service redesign, review and operational process change, cultural change and new legislative requirements.

Public health  

Herefordshire Council's public health team plays a major role in protecting and promoting health and wellbeing, preventing ill health and prolonging life. The team does this by commissioning a wide range of health services on behalf of local people such as:

  • Health visiting and school nursing
  • Smoking cessation
  • Alcohol and drug misuse treatment
  • Sexual health and contraceptive care
  • Infection prevention and control
  • Health promotion programmes, including obesity prevention and the health checks screening programme
