- I declare that to the best of my knowledge all the information I will provide is correct
- I understand that I must promptly inform Herefordshire Council of any changes that may affect my entitlement to a pass
- I agree to Herefordshire Council sharing information I provide on this form with other local councils or agencies responsible for the Concessionary Bus Pass scheme for the purpose of preventing and detecting crime
- So that we can process your application, you will be asked for personal information. All personal data held by Herefordshire Council, is processed in compliance with the principles of the Data Protection Act 2018
- Herefordshire Council works with Unicard Ltd and Experian for the purpose of processing your application and your information will therefore be shared with them. Unicard Ltd and Experian QAS Plc are fully compliant with the Data Protection Act 2018 and will treat all personal data in strict accordance with these guidelines
- Herefordshire Council works with Access Independent Limited for the purpose of assessing eligibility of applicants for disabled person’s passes and your information may therefore be shared with them. Access Independent Limited is fully compliant with the Data Protection Act 2018 and will treat all personal data in strict accordance with these guidelines
