Eligibility for disability services

For disabled children in Herefordshire we use eligibility criteria as part of the assessment to prioritise and ensure the families most in need receive appropriate help. A social worker will make a child and family assessment to decide if your child is eligible for specialist services, using the factors in the resource assessment matrix.

Request an assessment for disability services for your child

Referrals and requests for a service from the Children with Disabilities social care team need to be made by a health, education or social care professional with the parent's or carer's consent. These requests will be considered by a panel of professionals at the Child Development Centre

Following the child and family care assessment, we will use the information gathered in the assessment to decide if your child is eligible for support from the council for disability services. We will use the factors outlined in the resource assessment matrix to prioritise and to ensure the families most in need receive appropriate help.
