MPS: Our story so far

Together, we are creating a care and support system in Herefordshire that works for people at every stage of life. One that is rooted in community, focused on independence, and designed to support people to live well at home wherever it is safe to do so.

Herefordshire has a strong tradition of community support. Over time, we have brought together services across adult social care, community wellbeing, public health and housing to better reflect how people live their lives. This has allowed us to listen more closely to people, families and carers, and to design services that better reflect what matters to them.

People consistently tell us they want support that is:

  • Easy to find and understand
  • Joined up, so they do not have to repeat their story
  • Flexible enough to change as their needs change
  • Focused on supporting independence and connection

We have used this feedback, alongside data, research and national policy direction, to begin reshaping services. This includes strengthening prevention, working more closely with health partners, and investing in new models of support that better meet local need.

Our story is still being written. The next phase requires us to be clearer about how commissioning will actively shape the market, the workforce, and future models of care.
