1. Download the appropriate application form below.
   1. Option 1 application form if you wish to send us slides that you have created.
   2. Option 2 application form if you wish to send us text (and J-peg image if you can), which we will then input onto slide templates.
2. Fill in the application form and send it to Shirley Hill:

shill@herefordshire.gov.uk or by post to: Leominster Library, 8 Buttercross, Leominster, HR6 8BN.

• Applications must be sent at least 2 weeks before the slides need to be displayed.
• Costs and other details are available in the guidelines document below.
