Assessment Tools for Single Assessment Process
Contact Assessment – The Contact Assessment is the entry point for the older person to the Single Assessment Process.
A Contact Assessment should thus be undertaken for each new episode of care.
The form is intended to serve as a generic, receipt of referral document for both Health and Social Care Services.
It also serves as the front sheet for the Overview Assessment documentation (to be published here soon).
