About the Form
The form gives us the information we need to work out how much benefit you may get. We need to know a lot of information, so the form is quite long.
You may find some of the sections don’t apply to you, so you will not have to fill in all parts of the form. Every section starts with a question that helps you to decide if you need to answer all the other questions in that section.
Make sure that you answer all of the questions that you need to, by ticking or writing in the boxes on the form.
