The form gives us the information we need to work out how much benefit you may get. We need to know a lot of information, so the form is quite long.

Additional Forms for Changes to your Address
You can download or print off this following form to make an application for Housing & Council Tax benefit should you move within the Local Authority Area. There is also a Landlord and Agent form that can be printed off for your Landlord to complete should you be unable to provide a Tenancy Agreement.
