Assessment and Care Management

A worker from our Social Care staff will visit you and talk with you about your life and circumstances.  This will be written down and is confidential.  He/she will also want to find out what will help you to be more independent.  You may wish to have a relative, friend or carer help with this.  It also helps if we can involve others who contribute to your care.

If you have a carer, they are entitled to an assessment in their own right.

The assessment will show your needs and also whether you are eligible to receive a service from us.  If you are, we will work with you to write a Care Plan.  This will show what help you are to receive, who will give it and when.  We will work with you to put this plan into place.

If your ability to manage changes, you can ask for a further assessment.  We will review your situation regularly, to make sure that you are still getting the services you need.
