If you have a carer, they are entitled to an assessment in their own right.
The assessment will show your needs and also whether you are eligible to receive a service from us. If you are, we will work with you to write a Care Plan. This will show what help you are to receive, who will give it and when. We will work with you to put this plan into place.
If your ability to manage changes, you can ask for a further assessment. We will review your situation regularly, to make sure that you are still getting the services you need.
