Herefordshire Council and Herefordshire Primary Care Trust service have an integrated service for adults with a learning disability. This means that health and social care work together as one unit to provide services. The team includes social workers, community nurses, occupational therapists and clinical psychologists amongst others.
We can provide a range of services to people with learning disabilities and give support to carers. This could include providing supported living, planning for the future, or arranging short breaks to give everyone a rest.
To find out more about our services, click on the PDF link below in the Resources Box to read and download our leaflet. Or you can get a copy from one of our offices. You can also contact the team on (01432) 373200.
In 2001, the Government published its White Paper on Learning Disability Services called 'Valuing People'. It says that people with learning disabilities should have all the opportunities and services that society has, just like any other citizen. This means big changes for years to come involving many different agencies.
There is a link in the Resources Box below to Valuing People and a link in the External Links Box for an easy to read version.
All the news about Valuing People across the country and in the West Midlands is on the Valuing People Support Team website, which can be accessed through the External Links box below.
Every area in the country has set up a Partnership Board for 'Valuing People'. The Board's role is to make sure that the right changes happen to make Valuing People and the NHS Plan work in Herefordshire. Many different agencies, service users and family carers participate.
Local authorities have written plans for implementing Valuing People locally, and for meeting targets. In 2005, the Partnership Board wrote a Strategy to bring the plans up to date. It showed what the Board thought were the most important services for Herefordshire and what changes it could make in the following year.
The Board then produced an Annual Report in 2006 to show people how many targets had been met. New targets were then set in a document titled Partnership Board Plan No. 2 August 2006.
Please click on the links in the Resources Box for a copy of the full 2005 Strategy or a copy of the Easy to Read version. There are also links to the Annual Report and the Plan No. 2.
The Partnership Board have set up a monthly newsletter about learning disabilities issues in Herefordshire. There is an Editorial Board, mainly of local people with learning disabilities. If you would like a hard copy of the newsletter, please write to maggie.h26@ukonline.co.uk
