What is Person Centred Planning?
Person Centred Planning (PCP) is a collection of tools and approaches that helps put individuals and supporters in control of planning their own lives.
- It's about listening to you and what is important in your life
- It focuses on supporting you to achieve the things you want to do in your life and what really good support looks like to get you there
- It can support planning for your eligible needs if you are in receipt of funding
- It can also look holistically at your community and natural networks
- It is positive, values you as a person and helps tell people what you want! The plan belongs to you not a service
Who is it for?
Anyone can have training around the tools on this web site
The Wye Valley NHS Trust PCP team fundamentally works with people who have a critical or substantial assessed need, and:
- Have a learning disability
- A young person with a disability in transition in to adult services
Who can make a referral?
- Contact through a PCP team member
- Social worker
How does it work?
One of the team will meet you at your convenience, we will look at what you want to change or achieve in your life. It could be around:
- Moving home
- Making more friends
- Having a relationship
- Finding things to do in the day
- Getting a job
- Developing more skills
- Getting more control
During the process we focus on:
- Your gifts, skills, passions and qualities
- Your likes, dislikes and interests
- What type of support you want and need
- Who could support you best
