Registering with Shopmobility

Registration form

If you are a new user to Hereford Shopmobility, we will ask you to register with us. This involves filling in and signing a short form. You only need to register with us once.

You can complete the form when you arrive at the Centre or download one. We can, of course, assist with the completion of forms if required.

Proof of Identification

When you register with us, you will need to show some form of identification, which contains your name and address, such as a driving licence or utility bill.

Membership card

Once you have registered with us, we will give you a membership card, which we will ask you to show each time you borrow a vehicle.

Donations

There is no charge for registration or for the use of vehicles. However, there is a donation box if you would like to contribute to the costs of the Hereford Access for All charity, which supplies and maintains the vehicles.
