Services for People with a Physical Disability

Your Community Care Assessment will show which things you need help with. We will then work with you on a Care Plan, to plan just how and when the help you need will be given.

Examples of the kind of help we can arrange are:

  • Help with personal care, like washing and dressing
  • Meals
  • Support for carers
  • A place at a day centre
  • Adaptations or equipment in your home to make managing easier
  • Short term breaks for you and for your carers
  • If necessary, longer term residential or nursing home care

Some people receive payments from us (called Direct Payments), so that they can employ their own assistants to give the care which is needed.

You may also be able to have volunteer carers through the Independent Living Partnership scheme.

There may be a charge for some of these services. How much you are asked to contribute towards the cost will depend on your income, benefits and savings. We will work with you to complete a financial assessment and can then work out how much (if anything) you will be asked to pay.

More information is in our leaflet “Services for Older People and Adults with a Physical Disability”, which you can get from one of our offices. If you want to ask for a Community Care Assessment for yourself or someone you care for, please contact our Duty Social Worker.

We also have a separate guide to services for people with a Sensory Impairment. Please ask for a copy if you would like one.
