Transition planning

Transition means change from being a teenager to an adult.  It includes the change from children's support services to adult services and from school to adult life.
Herefordshire has developed a Transition Pathway to make sure that all young people with disabilities and learning difficulties are listened to.
It supports their achievements and aims in life, so that when they leave school they are full members of the community and lead the lives they want.
In order to do this we need to make sure that support is in place when they leave school.

Transition planning is a holistic, supportive, inclusive and evolving process which is carried out from Year 9 in school and each year thereafter.

If your son or daughter has a Statement of Educational Needs or Banded Funding levels 3 or 4, he or she should have a transition review in year 9 which is organised by the school.  The family are able to have people at the meeting who know the young person best. It will bring together the people who will ensure that you and your son or daughter can make plans for the future. 

Your son or daughter and the family will be given the chance to prepare for the school review meetings by using the Transition toolkit 'Getting Ready' sheets.  This will help everyone to think about a range of topics which will need to be considered to help plan for the future.  The school SENCo (Special Educational Needs Co-ordinator) will provide these for you.
