The survey, which has been randomly sent to one in 26 households, includes 42 questions covering waste, recycling, rubbish, public transport, cultural and recreational activities and venues as well as access to local services and anti-social behaviour.
Already 500-plus surveys have been completed and returned to Herefordshire Council.
The Council will use the results to improve its services and help raise the quality of life throughout the County.
The surveys should be returned freepost to the address provided by December 12. The results will also be made available to the public.
Cllr June French, Cabinet Member (Corporate and Customer Services and Human Resources), said: “This survey is important because it helps us tailor services to the needs of the communities we serve.
“I would urge everyone who receives a copy in the post to complete it and return it freepost to Herefordshire Council.
“Once compiled the information will also be shared among our partners so they too can use it as a valuable tool to guide their service delivery.”
